Transcript Requests

Transcript requests should be made using the form below. Please note that the fees for transcripts are as follows (each subsequent time that requests are made, the fees apply):

• $25 for the first official transcript (hard copy) mailed. $10 for any additional transcript (hard copy) mailed at the same time.
• $25 for the first transcript emailed. Free for any additional transcript emailed at the same time.

Once you have completed the form below you will be directed to a payment page to complete the process. After the payment is made, your request will be processed. Please note that transcript requests may take up to 5 business days to complete.
